REQUIREMENTS:

    * Photocopy of the applicant's authenticated birth certificate;

    * Certified true photocopy of high school diploma & form 137 (TOR)

    * Photocopy of the pages of a passport or any valid ID (driver's, postal, etc.)

    * Original prescribe Medical Examination form with doctor's certification

    * Original approved Pre-assessment Form

    * Two (2) pieces passport size pictures colored with white background

STEPS

  1. Get Enrollment Procedure and Pre-Assesment Form.
  2. Interview between parents (for those below 18) and/or applicants, together with the school administrator and/or guidance couselor and/or any of the senior faculty members. Fill out Pre-Assessment Form and get assessment of fees to be paid.
  3. Present the Pre-Assessment form to Cashier and pay the pre-assessment fees and down payment.
            *Note: The down payment secures a place in a class but does not guarantee admission
  1. Submit the Pre-Assessment Form and present Receipt to the Registrar to secure the Medical Examination form and the Written Test Permit
  2. Submit the Medical Examination Form to the medical clinic and undergo medical/physical examination. (To be scheduled)
  3. Submit the Written  Test Permit to the testing center and take the test. (To be scheduled)
  4. If the applicant fails the medical/physical examination, bring Medical Examination Form to Registrar to get approval for refund of down payment and anti-tetanus fee. Then proceed to cashier for refund.
  5. If applicant passes the medical/physical examination, get anti-tetanus shot and present the doctor certified Medical Examination Form to the Registrar. For applicants enrolling in non-basic welding programs, secure the Practical Test Permit for skills pre-assesnment. (To be scheduled)
            *Note: Anti-tetanus shot is prerequisite before taking the practical test.
  1. For applicants enrolling in non-basic welding programs, present the Practical Test Permit to the instructor and perform the tests.
  2. Wait for advise on the on the pre-assessment "approval to enroll" from the Registrar. If accepted, get the Training Agreement/Enrollment Form and the approved Pre-Assessment Form from the Registrar. If not accepted, get approval for refund of down payment. Then proceed to cashier for refund. (Release of test results will be scheduled)
  3. Fill-out and sign, and present the Training Agreement/Enrollment Form together with the approved Pre-Assessment Form to the Cashier and pay the corresponding tuition fee.
  4. Present O.R. for tuition fee and submit the Training Agreement/Enrollment Form with the following documents to the Registrar:
  • a photocopy of the applicants authenticated birth certificate;
  • a photocopy of high school diploma and Form 137 (TOR);
  • a photocopy of the pages of a passport or any valid ID (driver's, postal, voter's, SSS, GSIS, NBI Clearance) with picture that attest to the applicants identity
  • original prescribed Medical Examination Form with doctors certification
  • original approved Pre-assessment Form
  • two pieces passport size pictures colored with white background;
  1. Get your Class card and ID.
            *Note: Bring your Class Card and ID on the first day of training.
  1. Present your Class Card and ID to the Tool Room Keeper to claim your Personal Protective Equipment on the first day of training.
